Between the time the MMHD was authorized in 2006 and now, we’ve learned a lot about what works and what doesn’t work in Medicare care coordination programs. UHN...The video talks about the healthcare industry's dependance on outdated pager technology to coordinate complex patient care leads to inefficiencies, disruptions and other issues. The University Heath Network (UHN) wanted to lead the way in new approaches. All residents on the General Internal Medicine service at UHN have BlackBerry® smartphones. Advanced communication processes enabled by the BlackBerry smartphones were developed to improve coordination of care across the entire interprofessional team. This includes a web paging form that allows nurses to send the medical team text messages in a structured format, based on the urgency of the issue.
